This template should be used when two pages share the same name and will place a brief notice at the top of the page.
Usage:
{{otheruses|USE1|USE2|PAGE2}}
where USE1 is a brief summary of the current page (3-5 words at most), USE2 is a similar summary of the other page with this name and PAGE2 is the actual name (without link-brackets) of the other page.
Real world example:
{{otheruses|The Quest|The Item|Grandfathers Blade}}
is used to differentiate the quest and item both called Grandfathers Blade.
